IEQ-MP - Illness Effects Questionnaire - Multi-Perspective

The instrument is composed of four separate forms that assess a patient´s medical experience from multiple perspectives. IEQ-S is a self –report assessment in which the patient communicates his or her perception of the impact yhe illness is having on his or her life. IEQ-PRO is a questionnaire designed for the medical professional to report about the patient´s experience. IEQ-O is entended for someone close to the patient who is observing the impact that the illness and treatment are having on the patient´s life. IEQ-Tx, which, when completed by the patient, provides an assessment of the specific effects he or she is experiencing as a result of the treatment. A profile form, IEQ-CIP, allows a convenient means of monitoring illness effects over time from this multi-modal perspective.
